UnitedHealth Raises Outlook After Q3 Profit Beat, but Shares Fade

UnitedHealth beat third-quarter profit forecasts and lifted its 2026 earnings outlook after its medical costs fell. Shares jumped at the open yesterday but closed slightly lower, leaving the stock well below its July high.

UnitedHealth (NYSE: UNH), the US health insurer, beat third-quarter profit forecasts and raised its outlook, yet its shares closed 0.20% lower yesterday at $377.76. Adjusted earnings per share were $4.69, against a consensus of about $4.12-4.14 and $2.92 a year earlier.

The driver was cost control. Its medical-cost ratio, the share of premium income paid out on patient care, fell to 88.4% from 89.9% a year earlier, and beat the FactSet estimate of 89.7%.

Revenue was $112.7bn, against estimates of $111.4bn but down from $113.2bn, and operating earnings were $6.6bn.

Management now expects 2026 adjusted earnings of at least $20.50 a share. Zacks put the previous guidance at $19.50-$20, with the Street at $19.85 before the results.

The shares opened at $397.63, about 5% above the previous close of $378.52, and reached $399.88 before giving up the whole gain. The cause of the fade is not established, though healthcare was weak, with the NYSE Healthcare Index down about 1.1% in the afternoon. Peers barely moved: Elevance Health rose 0.75% and CVS Health fell 0.22%.

Part of the improvement was not repeatable. Investment bank Mizuho said the cost ratio beat consensus by 150 basis points, or 1.5 percentage points, but by about 80 basis points once about $610m of favourable prior-period reserve development is stripped out.

That still counts as a beat, and Mizuho kept an outperform rating and named UnitedHealth its top US healthcare pick. For traders, the question has shifted from whether margins recover to whether the recovery lasts.

The shares remain about 24% above their six-month closing low of $304.33 on the 10th of April, but well below their July closing high.
